Earlier Identification. Earlier Support. Greater Opportunity to Prevent Harm. The Aspira Harm Prevention Program is designed to help identify changes in wellbeing, behavior and risk earlier and connect individuals and families with appropriate resources and support before situations escalate. The program brings together CWM/MBC, education, personalized resources, care navigation, human connection and evidence based approaches to support prevention and earlier intervention. The program can be adapted for healthcare organizations, schools, universities, workplaces, community organizations and other populations.

Our Senior Well Being Check In Program offers gentle, ongoing check ins designed specifically for older adults. Families gain peace of mind, seniors feel supported, and care teams can respond early when support is needed. 

 Aspira is pleased to launch a new model to implement the Medicare Chronic Care Management (CCM) Program to enhance healthcare outcomes, reduce hospital remissions, and improve overall patient well-being. These services provide a proactive approach to managing chronic conditions, leveraging technology to offer continuous care outside of traditional clinical settings.  

Our Post-Discharge Follow-up program is for hospitals/facilities, providers, individuals and other organizations. We provide an additional follow-up program with resources, outpatient services, and technology tools to help improve the mental health status of patients after they leave a hospital or facility setting.

Aspira works with schools, universities and community partners to support student wellbeing, early identification, prevention and access to appropriate resources. Programs can combine wellbeing check ins, age appropriate assessments, personalized toolkits, resilience and life skills resources, behavioral health education, referral pathways and support for students, families and educators.

 The Aspira Care Navigator program helps providers and care teams to track and monitor a patient’s mental health status outside of in-person or live telehealth provider visits using our Measurement Based Care and Psychological Monitoring tools. Our approach to “whole health” monitoring, both physical and behavioral, results in fewer ED visits, more proactive care, better patient outcomes, and higher patient recapture.
